ER: Style, classDef and Class

Entities styled three ways, as mermaid 12.1.0 and 12.2.0 paint them (#2107): `classDef` with `class`, the `:::` shorthand, and an inline `style` (which wins over the class). `classDef default` sets the stroke of every entity. In the ASCII panel the colours are ignored, as in every other diagram type.
